Refractive Lens Exchange (RLE): When to Consider It

Refractive Lens Exchange (RLE): What It Is and When to Consider It?

RLE, or Refractive Lens Exchange, is a procedure for improving vision that can correct many refractive errors. It may be an option if you are unsuitable for laser vision correction and experiencing the age-related condition called presbyopia (the loss of the accommodative ability of the eye). Dr Ben LaHood is an ophthalmologist with a sub-specialty in refractive procedures and expertise in performing RLE.

What is RLE?

RLE, or also known as lens replacement surgery, is commonly suited to those over 50. This popular procedure is often considered as an excellent alternative for those people who are not suitable for laser vision correction.

In this blog, we explain how RLE works, who it may be suitable for, and what to expect from the process.

What is Refractive Lens Exchange (RLE)?

Refractive Lens Exchange (RLE) is a procedure where the eye’s natural lens is removed and replaced with an intraocular lens (IOL). While it shares similarities with cataract surgery, RLE is often carried out before cataracts develop and negates the need to have cataract surgery in the future.

It is used to manage refractive conditions like long-sightedness, short-sightedness, and presbyopia. The type of IOL used can be selected based on individual vision goals and clinical recommendations.

Who Should Consider RLE?

RLE may be suitable for individuals over 45, who:

  • Experience difficulty seeing up close due to presbyopia.
  • Have a prescription that falls outside the range for laser-based procedures.
  • Are looking for an option that reduces their dependence on reading glasses or bifocals.
  • Are not eligible for LASIK or SMILE® due to corneal thickness or other ocular factors.

A detailed ophthalmic assessment is required to determine suitability and the IOL that is best suited to the patient’s eye health and goals.

Advantages of Refractive Lens Exchange

RLE may offer the following outcomes:

  • Reduced need for reading glasses or bifocals
  • Lens selection tailored to personal visual requirements
  • No development of cataracts in the future, since the natural lens is replaced
  • Consistent visual results over time

These outcomes provide lifestyle convenience and visual comfort for those who are suitable and looking for a long-term visual solution.

Risks and Recovery Timeline for RLE

Although small, as with all surgical procedures, RLE carries certain risks, including:

  • Glare or halos, particularly in low-light conditions.
  • Mild discomfort or temporary dry eyes.
  • Risk of infection or inflammation, though these are uncommon.
  • Continued use of glasses for certain activities, depending on the lens used.

Recovery, although generally fast does vary between individuals. Many people notice improved vision within a few days, with further stabilisation over several weeks. Regular follow-ups and prescribed eye drops are a part of the aftercare process.

What to Expect During RLE Surgery

RLE is day surgery, performed in a clinical setting, and generally takes around 15 to 30 minutes per eye. The process typically involves:

  • A pre-operative consultation to measure the eye and discuss lens options.
  • Use of anaesthetic drops to keep the eye comfortable.
  • Removal of the natural lens and insertion of the selected IOL.
  • Discharged with aftercare instructions and scheduled reviews.

If both eyes require treatment, the second eye is usually treated after a short interval.

Refractive Lens Exchange (RLE) may be appropriate for people over the age of 50+ who are exploring vision correction beyond laser procedures or glasses. It offers an alternative approach for managing presbyopia or high prescriptions. As with all medical decisions, seeking guidance through a consultation with an ophthalmologist can help determine whether this procedure aligns with your needs and expectations.

Dr Ben LaHood is a specialist refractive ophthalmologist with experience and expertise in RLE. In consultation with him, you will be guided through individually tailored options and how to best achieve your unique visual goals.
